XD slide did not lock back...
I went to the range today and my XD's slide didn't lock back after the last shot TWICE. What causes this? I've only put about 250 rounds through it so far btw... thanks for your help!

Could be a weak mag spring; could be a misplaced thumb.

Yeah probably a bad mag, spring is not strong enough to push the follower up strong enough.

Hmmm... I'll try one of my other two mags and see if it keeps happening.

I had the same thing happen with one of my mags. No big deal.

Mags. My Glock went through as a period where it wouldn't lock open after the last round. Replaced my worn out mags and problem solved.
Not sure how the mags are that come stock with an XD, but the ones that came with my Springfield TRP are pretty cheap. If you buy replacement mags, I would encourage you not to buy Springfield mags. Buy some other type and rule out that it is, or it isn't the stock XD mags causing the issue.

Check your grip. This was happening with my P2000 and it turned out that sometimes my thumb was riding the slide stop just enough to cause the slide to not lock back.

+1 on the grip check. My usual 1911 style grip was letting my thumb hit the release/lock when I first started shooting my XD.
